Headwaters Health Care Centre has announced its plan to extend patient health care beyond the hospital walls.
The Headwaters2Home program (in partnership with Bayshore HealthCare Integrated Care Solutions) offers up to 16 weeks of comprehensive care delivered directly to patients’ homes by an integrated Bayshore team.
The process will help ensure a smooth transition from hospital to home, optimizing the patient experience.
Since 2023, the program has successfully served over 30 patients referred by Headwaters.
“The service, the people, and the health care workers – nurses, doctors – form an amazing team,” said Tom Porter, resident of Caledon and Headwaters2Home Client.
“I can only pray and thank them for everything they have done for us personally and what they endure daily. I’m looking forward to recovery, but it may take some time before I’m fully recovered.”
The program also addresses hospital capacity issues by allowing patients, particularly seniors transitioning to alternate levels of care, to receive necessary care at home post-discharge.
The program also offers 24/7 telephone support and conducts ongoing assessments for enrolled patients over a 16-week period.
